Kindergarten Registration

Kindergarten is a vital foundation for your child's future success.

Kindergarten Registration is ongoing. Your child must be 5 years old by July 31st, 2026.

Get started with enrolling your child by downloading/completing/signing these forms:

1. SIS-10W

2. Supplemental Kindergarten Enrollment Form

3. Home Language Survey

4. Kindergarten Student/Parent Info  Form

5. MV1 Form

We will also need the following items to enroll:

1. Original Birth Certificate

2. Valid Photo ID of parent/guardian

3. Proof of residence in the Kula area (bill with a service address in parent/guardian names)

4. TB Clearance

5. Student Health Record Form
