New Student Enrollment

NEW STUDENT ENROLLMENT

Welcome to Kula Elementary! We are excited to have you join our community. To enroll your child at Kula Elementary, please call our office at 808-876-7610 or stop by between 7am-4pm.

To get started on enrolling your child, we will need the following items:

1. Enrollment Form

2. Home Language Survey

3. Copy of your child's birth certificate

4. Valid Photo ID of parent/guardian

5. Proof of residence in the Kula area (bill with a service address in parent/guardian names)

6. TB clearance- For more info, please see School Health Requirements 

7. Student Health Record

8. MV1 Form
